Dakota County collects the following private information from you to provide you with library services:

All information you provide as part of the library card application (other than your name);
Information which links your name with materials requested or borrowed;
Information which links your name with a specific subject about which you have requested.

You are not legally required to provide this information to Dakota County Library, and there are no known consequences for providing Dakota County Library with this information. However, Dakota County Library cannot issue you a library card or provide materials to you if you do not provide the information.

This information is available to you, your parent/guardian if you are under age 18, Dakota County personnel, or other individuals pursuant to a court order, signed consent, or with statutory authority.

If you are under age 18, information about materials or subjects you requested or checked out is available to your parent or guardian. You may ask to have this information withheld by filling out a data privacy request form. Dakota County will make a determination whether withholding this information from your parent is in your best interests.

Additionally, Dakota County Library may release your reserved materials to your family members or other persons who reside with you and are picking up the materials on your behalf. You may request that reserved materials are only released to you by filling out a request to limit reserved materials form.

Responsibility Statement

I declare that I am eligible to apply for a library card. I agree to comply with all regulations, pay all fines, pay for the loss or damage of materials charged to this card, and give immediate notice of an address change or the loss of my card. A fee will be charged to replace a lost card. I understand that I am responsible for all materials checked out on this card and that my card and borrowing privileges are not transferable. Delinquent accounts may be handed over to a collection agency.
